Your First Visit
Our front desk staff will give you a friendly greeting and give you the necessary paperwork to fill out before your appointment. These forms will let us know if you’re suffering from any specific health issues that need to be addressed or taken into account. Once Dr. Whitmore is ready to see you, you’ll receive a routine examination followed by a thorough cleaning of the teeth and gums. We’ll schedule future treatments based on what was found.
New Patient Forms
There’s no need to wait until you get to our dental office to fill out the necessary forms; you can download them and print them out right here so that they can be completed with all of your information ahead of time. This simple step will dramatically cut down the amount of time spent in our dental office and allow you to see Dr. Whitmore that much sooner. Remember: these forms are absolutely vital for letting us know how to approach your care.
Dental Insurance Welcome
Having dental insurance makes it much easier to fit a trip to the dentist into your budget. We’re in-network with a number of providers, but we accept other plans as well, and we can help you examine your benefits so that you have a firm grasp on what services are covered under your plan as well as how much will be covered versus the amount you need to pay for yourself. Don’t hesitate to reach out to us if you have any questions about your insurance.
